What personal information do we collect?
Demographic and payment details are collected to provide a high quality of mental health services. The following information may be asked of you:
- Names, date of birth, addresses and contact details
- Medical history, current medication, social and family history and risk factors associated with mental illness
- Health Fund details
How do we collect your personal information?
Information is collected using the following methods:
- When your General Practitioner or other persons have referred you
- When you initially book an appointment and complete the client intake form
- Throughout the course of treatment personal information will be revealed and may be noted
- It is occasionally necessary to obtain information from third parties such as:
- Parents or Guardians
- Other health care providers such as psychologists, specialists, hospitals and other allied health professionals
- Your health fund or the Department of Veteran's Affairs
Who do we share your personal information with?
Personal information will only be shared under the following circumstances:
- With other healthcare provides if necessary (i.e. your GP)
- When a court has subpoenaed the information
- Under mandatory reporting laws (i.e. when you pose a threat to yourself or others, or when you are involved in a serious crime)
How do we store your personal information?
Information may be stored as either paper records, audio recordings or electronic files. All electronic information is stored securely on password protected devices. Hard copy information is stored in a secure office environment. Only approved staff members have access to these facilities. Confidentiality agreements apply to all staff.

Please address any concerns you may have about the privacy of your personal information to [email protected] and we will strive to resolve any issue in accordance with the Privacy Act 1988 and the Australian Privacy Principles (APPs).
Changes to Privacy Policy
Please note, it may be necessary to change our privacy act on occasion. Stirling Hypnotherapy reserves the right to do so at any time, at which point clients may be notified via email or through their website.
